Job Description Summary: |

- Assist residents in obtaining community
services to facilitate discharge
- Maintain statistical info in accordance
with state rules and regulations
- Identify and manage psycho-social needs
of the residents
- Provide on-going counseling to residents
and family
- Provide linkage to service, i.e. Social
Security, Dept of Social Services

| Requirements/ Experience: |
| |
- 2-3 years previous experience working
in Long-term care setting
- New York State Licensed Master Social Work.
(LMSW)
- Master’s Degree in Social Work
